We would love to know how many touchpoints, prayers, salvation, and connections are made every month.
This is an opportunity to provide honest feedback on a GoTeam Member or Leadership School Student's performance.
This information can then be used to identify areas where a person is excelling or areas needing attention.
REQUESTS AND FORMS
If you would like to request time off please fill out the form in the link.
Please submit 6 weeks in advance, avoid time off within 2 weeks of a Tier 1 event, and avoid time off on the First Tuesday or First Wednesday.

It will be submitted for evaluation and if approved, it will be put in the general calendar.
For maintenance requests or repair needs at Thrive please fill out this form.

Registrations are used to create online signups
to help you organize events, accept signup payments, and manage details for events.
For a request to use the church and its facilities for activities, not in direct relation to church events, training, or Go Team-related activities.

Planning Center is the software tool we use to help you organize information, coordinate events, communicate with teams, and connect with the congregation. ​
Example: People, Services, Forms and Groups
To request a Video to be recorded by the media team.
Videos can be for training, promotion, and internal and external use.
If this video is for training that will be online please ensure you have filled out the Learning World Request form prior to requesting the video.
Compassion Ministry exists to provide short-term financial assistance to tithing members on a case by case basis.
If your position requires you to maintain access or use personal access to the facility security cameras they can be requested with this form.
All requests are subject to approval and granted based on necessity and priority of use.
